1. General

These terms apply to all services provided by AffiniDerm, PLLC. We reserve the right to modify these terms at any time; changes will be posted on this page.

2. Eligibility

Our services are intended for clients 18 years of age or older. Minors require parental or guardian consent. All clients are required to accurately disclose relevant medical history prior to treatment.

3. Medical Disclaimer

Our treatments are offered for cosmetic and therapeutic purposes. They are not a substitute for medical advice, and clients with underlying health conditions should consult their healthcare provider before beginning treatment.

4. Laser Treatments

Laser hair removal, skin rejuvenation, and tattoo removal require sun avoidance for two weeks before and after treatment. Results vary by individual.

5. Facial Treatments

Facial rejuvenation, microneedling, and resurfacing treatments require a consultation beforehand and adherence to aftercare instructions.

6. Microneedling

Microneedling is a minimally invasive procedure that typically requires multiple sessions and specific pre- and post-treatment care for best results.

7. Hair & Makeup Services

Hair and makeup services should be booked in advance. Results vary based on individual factors including hair type, skin type, and event conditions.

8. Cancellations

We require 24 hours’ notice to cancel or reschedule an appointment. Members are not charged a fee for timely cancellations. Non-members who cancel with less than 24 hours’ notice forfeit their deposit.

9. Refunds & Payments

Payment is due in full at the time of service. All sales are final. Treatment packages are non-refundable.

10. Liability

AffiniDerm is not liable for adverse effects arising from undisclosed medical conditions, failure to follow aftercare instructions, or individual variation in treatment response.

11. Privacy

Your personal information is handled in accordance with our Privacy Policy, which describes what we collect, how we use it, and your rights regarding it.

12. Client Conduct

We reserve the right to refuse service to any client engaging in disruptive or abusive behavior toward our staff or other clients.

13. SMS Communications

If you provide a phone number, you may opt in to receive text messages from AffiniDerm, PLLC. This program is optional and is not a condition of booking or receiving services.

What we send. Our transactional program sends appointment confirmations, reminders, reschedule notices, and forms or paperwork needed for your visit. If you separately opt in, our marketing program sends promotional offers and updates. We do not send messages on behalf of third parties, and we do not text numbers obtained from anyone other than you.

Message frequency varies. Message and data rates may apply. Reply STOP to opt out of a program or HELP for help. Opting out does not affect your ability to book or receive services.

Consent language. Wherever we collect SMS consent, the checkbox reads: “I consent to receive transactional text messages from AffiniDerm, PLLC at the phone number provided, including appointment confirmations, reminders, reschedule notices, and forms or paperwork needed for your visit (optional, not required to book an appointment). Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to opt out.”

Carriers. Mobile carriers are not liable for delayed or undelivered messages. Delivery is not guaranteed and depends on your carrier, device, and coverage. Any carrier charges are between you and your carrier.

14. Governing Law

These terms are governed by the laws of the State of Nevada.
